More About Colfax Manor Assisted Living

Licensed, small, memory care facility on the South Side of Chicago. Our facility provides private and semi-private accommodations for residents with dementia in a communal setting, that allows for friendships and maximum socialization. Our dedicated staff receive yearly 20 hours of on going dementia training. Our staff provides 24-hour care with bathing, grooming, medication assistance, daily meaningful activities, and nutritional meals with snacks in-between. Our facility has an In-house physician that provides monthly visit and as needed for residents. Our nursing staff provide weekly nursing care and as needed. Our medical staff work with the residents’ primary care providers to ensure continuation of care.
