Concord Park has partnered with Vantage Healthcare to provide onsite primary care and care coordination services to our residents. Now, residents don’t need to leave the community to access Vantage Healthcare’s Care Team, including a lead Physician (MD), Nurse Practitioner (NP), and Behavioral Health Physician Assistant (PA). Residents can also enroll in Chronic Care Management (CCM) and receive the services of a Nurse Care Coordinator. This is a Medicare program offering concierge-level service. The Vantage Care Coordinator would also be responsible for coordinating care with other health care services, including Home Care, Rehab, Pharmacy, Lab, Specialists, the Hospital, and more! Reach out to us to learn more about this opportunity for Concord Park residents and families.
