Whether you need post-acute care after an unplanned hospitalization or recovery assistance after a planned surgery, Oregon City Health Care Center is here to support you with all your clinical and rehabilitation needs. Our Therapy teams are committed to providing an individualized treatment plan with the goal of helping each patient get back to doing the things that are most important to them.

Our Clinical Teams are skilled and experienced in assisting patients in a wide range of diagnoses while offering continued support throughout the healing process. Our Clinical Team is comprised of a Physician, Nurse Practitioner, Registered Nurses, Licensed Practical Nurses, Registered Dietitian, and Certified Nursing Assistants. In addition to short-term post hospitalization care, our center offers specialty skilled nursing and long-term/ongoing care when a patient’s care needs are no longer able to be met at a lower level of care.
