Our Victorian-style community is quiet and comfortable, and features a lovely walking trail surrounding the entire property, as well as a garden and trail within our secure Reminiscence Neighborhood. Our residents and families enjoy the serenity of the outdoors and often use the spaces for relaxation and conversation. Within our community, it is the professionalism of our caregiving team that sets Sunrise at Five Forks apart. Our highly trained team provides care that is individualized and based on each person's unique needs and preferences, and we provide a continuum of care that helps residents age in place. A signature of all Sunrise communities is the robust programming that includes a variety of classes and activities each day. Our residents in particular enjoy our Friday afternoon socials, which bring many of us together for laughter and friendship. Our dining program is also exceptional; our chef prepares local, seasonal meals from scratch each day, including Southern fried chicken--a resident favorite. Our Sunrise residents and staff are active members in the greater community. Once a week, our residents perform “Random Acts of Kindness” throughout Gwinnett County, including passing out small gifts with positive messages attached. This brings joy to everyone within the Five Forks community. We are devoted to providing our residents support and services, based on research and theories of aging—working hard to fulfill our mission to champion quality of life for all seniors. Our founder, Paul Klaassen said, “The best senior living has yet to be invented,” and we take this to heart. To us, this means committing to continuous quality improvement by learning from our past experiences and current residents and families.
