West Anaheim Extended Care is a skilled nursing facility that provides a full range of services to meet each individual’s personal goals and needs. The highly skilled nurses provide the best quality care for residents requiring IV therapy, dementia care, wound care, respite stays, enteral feeding program, catheter care, colostomy care and ileostomy care. They also offer hospice and restorative nursing services. The rehabilitation team is committed to creating individualized therapy programs based on personal goals with physical therapy, occupational therapy and speech therapy offered as part of the program. Outpatient clients receive treatment from the same therapists throughout the entire program with an emphasis on maximizing independence and ability.
